Thread health
Every message you send re-sends the whole conversation behind it. So a thread that has run long burns through your usage faster and holds onto less of what you decided in it. Past roughly 30 messages you start to feel it: questions you already answered, decisions quietly dropped, drafts getting thinner.
A fresh thread fixes both. The ledger and the handoff are how you start one without losing the work. If you ran step 7 of the setup checklist, your workspace already warns you when a thread is getting heavy.
After anything you settle, say log it into the ledger. Decisions, not tasks.
Say backfill, then handoff. Backfill sweeps the session into your ledger. Handoff writes the full context to a file in your workspace and hands you back a single title line.
Paste that title line as your first message, so the thread takes the name. Then send load my handoff. Claude reads the file and picks up where you stopped.
Backfill first, always, then hand off. The ledger is the part that survives the thread.
